When the time comes to be discharged from the hospital, our caregivers are available to provide support as you transition back to your home or a short-term recovery facility. Global Health Care Services post hospital care is always flexible to meet the family’s needs, and services can be increased or reduced as needed during the recovery process. Additionally, Global Health assigns a Care Manager to each of our clients who goes to great lengths to ensure the client receives high quality, consistent care throughout the recovery process.

The recovery period after discharge is a crucial time in the recovery process. To ease a transition from hospital to home or long term care facility, Global Health’s assistance begins in the hospital. We will advocate on your behalf by working with the discharge planner to ensure your care plan is in place and any government or insurance covered services are ready and available for when you are discharged. Our caregivers will assist you with navigating the healthcare system, ensuring you stay fully informed and at ease every step of the way. They will also help to ensure any supplies or equipment needed are available and can oversee any home modifications that are necessary to keep you safe at home.

Once discharged from the hospital, our caregivers are dedicated to providing high quality assistance, compassionate care and can help with things like housekeeping, meal preparation, errands, wound and other nurse led care, accompanying the family to follow up appointments, ensuring medications are taken as required and offering peace of mind to family in the form of respite care.
